About the role
Tweed Community & Allied Health have a career opportunity for a Registered Nurse to work with members of the Palliative Care nursing team (including CNCs and CNS’) across the inpatient and community settings. The aim of the Palliative Care Nursing service is to facilitate a coordinated, high quality Palliative Care service to the local community, with a case coordination model that involves generalist community nurses in providing direct clinical care.The service supports GPs, Community Nurses and the Residential Aged Care In-Reach Team in the assessment, care and management of patients with life limiting illnesses and takes a multidisciplinary approach to care with the end goal of improving quality of life for those patients and their carers.
