Sentinel Community Services is an Aged Care service and Registered NDIS Provider based in The Manning Valley, Sentinel works to enable Seniors and people with a disability to live independently and without limitations while achieving their goals.
We are seeking Support Workers for Part Time position to service our clients In the Mid North Coast Area.
Role Requirements:
- Registered with AHPRA.
- Maintain nursing registration by meeting CPD expectations and pursuing personal self-development.
- Minimum of twelve months post graduate experience.
- Experienced in caring for both mental health and generalist clients.
- Good communication skills with a variety of work colleagues, clients with differing abilities and their significant others.
- Ability to work harmoniously within a team.
- NDIS worker check.
- Undertake Working with Children check if required.
- Current drivers’ licence with own car.
- Australian citizenship.
Role Responsibilities:
- Initial assessment of all new clients and provision of a report to Management.
- Development of care plans, based on client needs and preferences, to provide the best possible care and advice.
- Act as Case Manager and direct other Support Workers in care.
- Periodic review of care plans based on changing client needs and preferences.
- Monitoring client condition by means of wellness checks as required.
- Liaise with Management and Support Workers on client needs, care plans, and any changes to same.
- Liaise with client’s General Practitioner where necessary regarding changing client needs.
- Liaise with other members of multidisciplinary teams as required.
- Providing professional advice to clients and their significant others.
- Perform routine nursing procedures, for example measurement of vital signs, medication administration, injections, dressings, care of catheters and care of PEG tubes to name a few.
- Monitor the work of Support Workers and ensure that duties are carried out to a high standard and in accordance with Sentinel Community Services Policies and Procedures.
- Provide assistance and training to Support Workers as required.
- Provide immediate care in emergencies.
- Ensure medical records are accurate, meet legal requirements, and align with Sentinel Community Service Policies and Procedures.
- Practice within WHS guidelines and established practices.
- Participate in continuous quality improvement activities.
