About the Role
The purpose of the role is to ensure the best outcomes for our participants by navigating and facilitating the access, management and cohesion of services and supports following discharge from hospital and during their time of adjusting and settling into community life following serious injury. It will involve establishing, coordinating and monitoring multi-disciplinary community-based rehabilitation programs with a focus on maximizing the participants recovery and independence.

Duties
It’s an exciting time at icare where no two days are the same; as a Rehabilitation Case Manageryou will:
- Use a person-centred approach, support the participant to develop a My Plan to maximise their involvement in planning through self-assessment, self-reflection and self-identification of strengths, priorities and goals in alignment with KPI’s of the role.
- Guide the participant and their families in setting expectations, understand what they can reasonably expect from Care and Community and to foster a life-long relationship that is built on trust.
- Assess and actively monitor the effectiveness of selected strategies and supports in meeting goals and steps throughout the My Plan period.
- Ensure appropriate selection of supports in subsequent plans and to ensure the participant is making future decisions based on comprehensive information about effectiveness of past choices as supported by internal specialist services.
- Apply critical reasoning and thinking within a decision-making framework within the team and in collaboration with internal specialist services and use of resources when establishing and coordinating requests for services and supports to ensure they appropriately meet the needs of people with severe injuries in terms of their progression along the rehabilitation continuum.
- Approve recommendations and funding regarding reasonable and necessary treatment, care and support within agreed functional delegations
Your story so far:
- Tertiary qualifications in an allied health, or other relevant discipline and/or demonstrate at least 5 years of full time (or equivalent) work experience which has equipped you with transferrable skills to provide case management of people with disabilities similar to those supported by our schemes.
- Experience and/or understanding of Lifetime Care, CTP/Workers Compensation legislation and guidelines
- Capacity to work full time
- Highly developed understanding of the needs and perspectives of people with disabilities.
- Critical reasoning skills to ensure a high degree of integrity in the decision-making processes.
- Flexibility and insightfulness and an ability to tailor established service delivery to meet the circumstances of each participant/worker.
- Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.
- A range of experience working in relevant health or disability areas.
- Knowledge of the service system for people living with a disability.
- Knowledge of a person centred approach to service delivery.
- Experience working with people with brain and/or spinal cord injuries.
- A current driver’s licence, with the ability to travel.
- Maintain a current Working with Children Check.
- Up to date with current Public Health Order requirements
