About the Role
- Coordinate and perform a defined range of clinical screening assessments, for older patients with complex care needs, as prescribed and delegated by the Eat Walk Engage team, and as allowed by legislation and guidelines.
- Liaise and work effectively in a professional manner with clients and their carers, other health professionals, students, volunteers and external agencies to achieve a client centred approach, utilising negotiation and conflict resolution skills as required.
- Provide direction, supervision and support to multi-professional assistants or volunteers where appropriate.
- Support Eat Walk Engage in the coordination and implementation of training, and staff development, to address general operational and system challenges, and improve services to enhance teamwork and patient centred care, in line with evidence.
