The DepartmentThe
Aged Psychiatry Service (APS) provides assessment, treatment and ongoing management for people over the age of 65 years with a mental health disorder including psychological & behavioural disturbances, for persons residing in the Inner South East catchment area.The APS consists of Baringa which is a 15-bed acute inpatient unit as well as the community based Mobile
Aged Psychiatry Service (MAPS), a hospital-based Consultation & Liaison service (including off campus Transitional Care Program (TCP).
The RoleAs one of the Senior Case Managers, you will be responsible for intake/triage, case management and/or consultation & liaison.You will have well developed organisational skills and will provide a high standard of case management to clients and expert liaison with other stakeholders in keeping with relevant legislation, policies and procedures. You will exhibit a high standard of clinical care delivery and be a role model to other junior team members and students.
Qualifications and Experience RequiredEssential- Approved tertiary qualifications and eligibility for membership with relevant registering body (Nursing and Midwifery Board / Occupational Therapy Board via AHPRA, or eligibility with AASW)
- Postgraduate qualification(s) in Mental Health
- Four years' experience post registration, to include clinical experience in acute psychiatric setting(s), and at least two years community psychiatric nursing experience
- Experience and/or willingness to undertake telephone referrals and determine service response - desirable
- Current Victorian Driver's License (without restriction)
- Australian working rights and full COVID-19 and Influenza vaccination
- Be willing to undergo a Background Check
