Core responsibilities of the Service Coordinator role include:
- Support, coach, train, mentor teams to build capacity and competence in the provision of Participant-centred supports and service delivery
- Identify and proactively resolve operational challenges as they occur
- Build and lead strong, collaborative relationships with internal teams, Participants, stakeholders, government agencies, allied health professionals, Support Coordinators and other parties
- Lead a service delivery culture centred around continuous improvement
- Ensure all service delivery to all Participants meets NDIS guidelines, industry codes and is compliant with relevant laws
- Be involved with the intake process of new Participants and ensure the delivery of supports meets all relevant stakeholder requirements
- Work closely with all Participants and Support Workers to ensure that supports align with Participant desired outcomes and NDIS plan
- Be responsible and accountable for service delivery of all Participants
Core requirements of the Service Coordinator role include:
- Qualification in Disability Support, Community Health, Mental Health or relevant discipline, or well developed experience in a Service Coordinator role within the disability sector
- Strong knowledge of NDIS guidelines, service excellence practices and industry codes
- Exceptional interpersonal, communication & people leadership skills – you’re a natural coach/leader who can lead, coach, train support team members to ensure they perform their best and deliver with excellence
- Demonstrated understanding and ability to deliver services to Participants with various needs
- Strong attention to detail, able to solution generate
- Passion for supporting people with disabilities with respect and trust
- Ambitious, growth-orientated, pro-active individual
- Effective problem solver and decision maker
Essential requirements of the role:
- NDIS Workers Clearance (must have or be willing to get)
- National Police Check (must have or be willing to get)
- Working With Children Clearance (must have or be willing to get)
- Current CPR & First Aid Certification (must have or be willing to get)
- Covid 19 Vaccination Status
- Full unrestricted Driver’s license and vehicle fit for purpose
