Due to growth in Community Care we are seeking a passionate and person-centred person to join our dynamic team.
We are looking for an experienced,professional,dedicated and caring person to work with a diverse range of clients who will be the main contact for clients and support workers.
The successful applicant will support and motivate support workers to establish and maintain a supportive and engaging environment with clients and team members.
This role is suitable for a solution-focused multitasker and a team player with high energy, resilience, and positivity.

Your role includes:
- Assisting clients with maintaining their rosters of support to meet their individual goals.
- Manage client information to ensure all relevant records and correspondence are current and provide an accurate reflection of service provision.
- Coordinating meet and greets for choice and control.

Our Applicant will have:
- An engaging Person-centred approach to Support Services
- Exceptional communication and relationship building skills
- A genuine desire to make a difference in the lives of people living with disability
- Have an exceptional understanding of support services, geographical locations, and essential services.
- A one team approach with the internal team
- Relevant and proven experience in disability, ideally in the provision of services with an understanding of complex care
Skills & Experience Required:
- Current DHS Disability Clearance or NDIS Worker Screening Check
- Valid Unrestricted Driver's Licence
- Strong computer skills
- Clear communication skills
- Experience with Time Target highly regarded
