As a Disability Care Coordinator, you will be responsible for ensuring that our patients receive high-quality care, with a focus on timely access to healthcare services and support throughout their treatment.
Responsibilities:
- Coordinate care for patients, including scheduling appointments and arranging for diagnostic tests.
- Communicate with healthcare providers to ensure timely follow-up and coordination of care.
- Educate patients and families on healthcare services and treatment plans, including information and referrals to community resources.
- Assist patients in navigating healthcare systems and understanding their treatment plans.
- Collaborate with insurance providers to ensure proper coverage for patient care
- Participate in quality improvement activities to improve patient care.
- Advocating for clients' rights and needs and working to address any issues that may arise.
- Working in partnership with a diverse group of professionals to deliver seamless and well-rounded care to clients.
- Maintaining accurate and up-to-date client records and reporting any changes or concerns to relevant parties
- Participating in continuing education and training opportunities to maintain knowledge and skills.
Requirements:
- Minimum of 2 years of experience in a healthcare setting
- Knowledge of healthcare services and medical terminology
- Excellent communication and interpersonal skills
- Strong organizational skills and attention to detail
- Ability to work in a fast-paced environment
- Proficient in Microsoft Office and electronic medical record systems
- Ability to work independently and as part of a team
- Understanding of relevant laws, regulations, and community resources is desirable
