About the role
- Provide advanced social work services and clinical leadership to the multidisciplinary teams around Long Stay patients where highly complex psychosocial needs are identified and could potentially impede a timely discharge.
- This position provides independent clinical services across the hospital and may be required to participate in an on-call service.
