About the role
An opportunity exists for a Grade 3Social Worker to join the Eastern Health Team in the GEM@HOME/REHAB@HOME sevice. This service is a bed substitution model and has received funding to expand the service to one the biggest in the state with 43 bed substitution beds. The team provides high quality flexible health services to patients in their own homes that require short term directed care at home.
This role entails operational leadership of a dynamic interdisciplinary team, clinical oversight of the GEM@HOME/REHAB@HOME social work service and provision of high quality care of the clients in this cohort. As the Grade 3 Social work lead you will provide clinical supervision to grade 2 Social Workers. You will also receive clinical supervision from a Grade 4 Social worker and become a member of the Eastern Health Social Work senior leadership team, including opportunities to participate in innovative quality and research projects.
This is a fantastic opportunity for an enthusiastic, experienced, passionate and dynamic Grade 3Social Worker to join the expanding GEM@Home/REHAB@HOME program, to deliver early rehabilitation service in the home as part of a multidisciplinary team. As the successful applicant, you will lead the social work team to deliver great patient care and promote an integrated service that is consistent with the mission and values of Eastern Health.
About you
To be successful in this role you will possess:
- Eligibility for membership of the AASW
- Evidence of current CPE, as per AASW requirements
- Highly developed interpersonal and leadership skills
- Experience in supervising other Social Workers
- Conflict resolution skills
- The ability to lead complex discharge planning
- Excellent time management skills
- A strong commitment to high quality care
- The ability to work of and lead a diverse team and also work autonomously as required
- Current Victorian Drivers licence
