Social Worker - Senior

Refugee Health, Chronic Disease Stream are looking for a Social Worker Senior – Refugee Health to join their Logan Community Health Centre team. Delivering high quality evidenced based Social Work clinical services to patients/consumers in a community care setting within the context of the Nurse lead Refugee Health model of care.

About the Role

• Acts as a clinical reference point for social work related matters with the Refugee Health team and responds to every-day clinical demands.
• Collaborate on and undertake the development of evidence-based clinical practices, policies and protocols within the Metro South Health Refugee Health Service.
• Apply increasingly independent clinical judgement to Social Work complex decision making with only occasional clinical/professional supervision.
• Adapt routine clinical practice as required based on clinical evidence with minimal supervision and a high degree of independence.
