About you:
In your new role you will…
- Have proven ability to deliver high level contemporary clinical Social Work services in sub-acute Older Persons and adult rehabilitation services as a member of an interprofessional team.
- Demonstrate an understanding of the legislation and policies relevant to the work particularly in relation to advanced care planning, guardianship and administration matters, residential aged care and community care, and NDIS.
- Have experience in the provision of supervision to Social Workers and the ability to meet accountability requirements in reporting / advising Social Work management team of relevant professional issues identified within their supervisory role.
About your opportunity:
In this position you will deliver high level Social Work services as a member of the interprofessional team to adult and older patients of Cairns Sub-acute Services and high-level clinical advice to colleagues within the CHHHS. When required, on occasion, deliver social work services to other OPSAR services, hospitals and locations in CHHHS. Clinical work undertaken will be of a complex and varied nature.
About your role:
- Deliver high level Social Work clinical services across a complex case load within the Sub-acute services and other clinical areas when required in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW professional practice standards as a member of the interprofessional team. This shall include but is not limited to psychosocial assessments, interventions, and subacute case management, ensuring patient focused outcomes and balancing these with the available resources and the operational needs of the health service for the benefit of the wider community.
- Apply independent high level Social Work clinical decision-making skills integrating evidence-based practice to contribute to multidisciplinary complex case management and discharge planning to achieve best practice outcomes with infrequent need for direct clinical practice supervision.
- Provide clinical practice supervision and mentoring of HP3 Social Workers, students and clinical support staff to ensure the maintenance of professional clinical standards within the social work services and multidisciplinary teams in the local facilities.
