About the role
We are seeking a dedicated Specialist Family Violence Refuge Practitioner for our Case Management and Outreach Services Program! You will provide Case Management support to victim-survivors who have engaged in supported accommodation services through refuge and transitional housing as well as provide an outreach response. Additionally, you will provide delivery of intake into the Program and targeted interventions determined by the Family Violence risks and needs assessments.
Responsibilities include
Deliver intake across short-term refuge, long term refuge, transitional housing and OutreachCase Management
Case management support to victim survivors in accordance with TSA Case Management models, standards of practice and Case Management Program Requirements
Provide Case Management support to victim survivors pre-entry into supported accommodation and support recovery
Victim survivors are assisted to access holistic support services
Key performance indicators are met in accordance with outcome measurement timeframes
This is a permanent full-time position, based in Coburg North, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
What you will have
Tertiary qualification in Social Work/Social Sciences or related field (essential)
Qualifications must align with the Mandatory Minimum Qualification Requirements for Specialist Family Violence Workers (Recommendation 209)
Experience working in Family Violence sector (desirable)
Experience in Specialist Family Violence and/or child well-being work (highly desirable)
Demonstrated experience and skills in intake, Case Management and crisis responses
Highly developed time Management and work prioritisation skills with an ability to work autonomously with minimal supervision required
