The Role:
The Specialist Support Coordinator plays a pivotal role in delivering comprehensive support to participants facing complex needs, challenging behaviours, medical requirements, or high-risk situations. Your responsibilities encompass crisis resolution, risk management, facilitating informed decision-making, and guiding participants in accessing a diverse range of services and supports. You'll collaborate with mainstream and community services, aiming for seamless integration of service delivery while prioritizing the reduction of complexity in participants' support environment. Your focus extends to overcoming barriers and enhancing connections with broader support systems and funded supports.
Role Key areas will include:
- Conduct comprehensive assessments of participants' needs and goals.
- Develop, implement, and review NDIS plans to ensure they align with participants' aspirations.
- Liaise with various service providers and community organizations to connect participants with appropriate support.
- Provide guidance and advocacy for participants to access NDIS-funded services and programs.
- Monitor and report on the progress of participants' goals and make necessary adjustments to their plans.
- Maintain accurate records and documentation in accordance with NDIS guidelines.
- Keep up-to-date with NDIS policy changes and ensure compliance with all regulations.
Requirements:
- Tertiary qualifications in Social Work, Disability Studies, Education, Nursing, Physiotherapy, Occupational Therapy, or equivalent would be considered.
- Specialist Coordination experience 3 years minimum
- Proven track record of achieving good results from the NDIS and healthcare funding partners that are aligned with the clients best interest
- Sound understanding of relevant legislation, NDIS procedural guidelines and the full range of community services that may be required by participants
- Ability to work independently, think innovatively and demonstrate initiative
- Highly developed written and verbal communication and negotiation skills
- A sense of urgency, excellent organizational skills and multitasking capabilities
- An ability to challenge current paradigms in healthcare and disability care
- All appointments are subject to NDIS Worker Screening Clearance.
