HIRING: Specialist Coordinator / SupportCoordinator

Futura Group ANZ Pty Ltd is an NDISdisability support service with a difference. We offer a range of supportservices to individuals who are part of the National Disability InsuranceScheme (NDIS) and their families.
We are currently looking totake on a full-time Support Coordinator / Specialist Support Coordinator for ourparticipants to support them with in-home support, servicing the GreaterMetropolitan Melbourne and surrounding areas.
JOB DESCRIPTION
The NDIS WorkforceCapability Framework describes this role as part of Additional Capabilities forservices
funded under the NDIS andapplies to NDIS Support Coordinators. The Support Coordinator plays an
important part insafeguarding a participant's well-being.
The Support Coordinator hasa responsibility to:
• Addressing any concernsor issues raised by participants regarding the quality and safety of theirsupports and services.
•Ensuring participantwell-being.
• Assisting participants inoptimizing their funded supports by helping them understand their plan's scopeand available resources.
• Act as advocates forparticipants, ensuring their rights and preferences are respected.
It is essential that the Support Coordinatorpossesses the following qualifications, licenses, and/or certificates:
• Current First Aid level 2 Certificate
• Current CPR (annual)
• Current National Police Check (less than 6 monthsold or evidence of application)
• Current Working with Children’s Check (WWCC) orevidence of application
· Current NDIS Worker Screening or evidence of application
• Evidence of Right to Work in Australia
• Current 100 points of Identification (we cannotaccept expired IDs)
• Current Australian Driver’s License
· Tax File Number
• NEW NDIS Worker Induction Module certificate
• DSC – Supporting People to Stay Infection-Free (COVID-19) certificate
