Hall & Prior Health & Aged Care Group is a family-owned aged care provider operating 36 residential aged care homes, with in-home and community care programs in WA and NSW. We are currently seeking a Specialist Support Coordinator to join our From Hospital to Home (FH2H) unit in South Perth, on a full-time basis.
The FH2H unit is a purpose-built facility that houses and supports 12 participants at a time to build skills and capacity to live as independently as possible in the community. The FH2H team has dedicated support workers, administration, clinical & allied health support and unit management to allow you to build rapport and understand individual support needs to develop evidence to support funding requirements.
As Specialist Support Coordinator, you will manage and support the transition of hospital patients with a disability, fit for discharge to source accommodation and funding most suitable in meeting their needs.
The FH2H program is a WA Department of Health funded initiative.
The role would suit candidates with vast experience navigating NDIA funding systems, case managing participants living with a disability who seek permanent housing and supports to live their life the way they choose.

Minimum cert 4 in Community Services/Disability/Mental Health or alike
NDIS Worker Screening Check
NDIS Worker Orientation Module Quality, Safety and You
evidence of Influenza vaccination
