Support Coordinator

The Opportunity:
ConnectGV are seeking a dedicated and experienced Support Coordinator who will be responsible for providing support coordination to our participants and facilitate the implementation of all supports identified.
Core Responsibilities:
As a Support Coordinator, you'll be a crucial team member, helping participants access the support they need through their NDIS plans. Your role involves building relationships, managing diverse caseloads, teamwork, efficient service delivery, NDIS compliance, and promoting ConnectGV services. Quality assurance and compliance are key, encompassing NDIS Practice Standards and incident reporting. You'll support participants in understanding their NDIS plans, coordinating supports, connecting with services, and maintaining client supports, all while ensuring budget monitoring and NDIA reporting.

Qualifications & Experience:
The ideal candidate will hold a qualification in Disability, Health, or Community Services, possess strong skills in influencing, negotiating, facilitation, and networking, excellent interpersonal and communication abilities, sound strategic thinking and planning skills, and demonstrate experience in leadership, coaching, person-centered practices, and financial management, coupled with attributes such as innovation, determination, self-discipline, analytical thinking, resilience, inclusivity, client focus, collaboration, and supportiveness.
Successful candidates will need a satisfactory NDIS Worker Screening Check, National Police Check, and Working with Children Check.
