Reporting to the Support Coordination Leader, you will contribute and provide holistic and person-centred support to our customers. As our new full time Support Coordinator, you will work alongside customers who live with Multiple Sclerosis, Motor Neurone Disease, and other progressive neurological conditions to connect them with the services and support they need to live an empowered life. Join our SPARK NeuroCare team to deliver adaptable, diverse, and accessible service for our customers.
In this full-time permanent role, work 5 days a week from your own home office based in Brisbane South or Brisbane West with some customer and stakeholder meetings in the community.
Additionally, you will:- Partner with customers and their families to identify goals, needs and opportunities
- Develop and maintain strong relationships across the sector to enhance customer choice
- Provide NDIS Support Coordination and create opportunities to improve quality of life
- Drive change and help grow a culture of collaboration, respect, and continuous improvement
- Qualification or equivalent experience in disability or related human services fields
- Highly developed and engaging written and verbal communication skills
- Working knowledge of the NDIS and My Aged Care service environments
- Networker able to navigate complex challenges within a community setting
- Exceptional ability to self-manage
- Team player with a down-to-earth, energetic approach
- A NDWS clearance, Blue Card and completion of NDIS Online Worker Training Module will be required for this role (prior to commencement)
