Experienced in supporting the needs of people with complex disability and/or Mental Health you will be motivated by seeing these individuals thrive within their communities as a result of care plans that promote independence, resilience and confidence. These skills will have been gained in a role with similar case management responsibilities where you have supported a portfolio of clients in helping them to access supports and services within their community.Additional attributes we are looking for include:
- An understanding of NDIS Framework and Support Coordination
- Knowledge of accessing community resources, particularly in the areas of health, drugs and alcohol, legal, income support, employment and recreation
- Passion and commitment with a flexible approach
- Effective written and verbal communication skills
- Emotional resilience, patience and determination to get the job done
- Conducting needs assessments based on their NDIS plan and goal
- Develop a support plan that identifies clear actions
- Enabling the client to connect with various service providers
- Assist clients to achieve goals through building capacity, knowledge, resourcefulness and confidence
- Provide support to the families of clients
- Work with local service providers and community members
- Build and foster positive ongoing stakeholder relations
- Prepare case notes, reports and updates as required by ERMHA365 and the NDIA
