Catena Programs and Services Pty Ltd is a registered National Disability Insurance Scheme (NDIS) service provider seeking an enthusiastic, highly skilled and motivated individual to fill the above position in our Support Coordination Team. We at Catena deliver services nationally, with a strong focus on providing person centered services guided by our Support Coordination team. Qualifications & experience
- Eligibility to work in Australia, NDIS Worker Screening Check, Working With Children Check, First Aid Certificate, valid Australian Driver Licence and access to a fully insured vehicle.
- Demonstrated understanding of mainstream support services.
- Demonstrated understanding of the NDIS Support Coordination framework.
- Tertiary level qualifications in a relevant discipline (e.g. Disability, Individual Support, Community Services, Mental Health) or extensive/relevant experience in case management, counselling and or case work with families or people with a disability.
- Excellent organisational skills, accuracy and attention to detail.
- Demonstrated knowledge of NDIS support items, plans and the concepts of choice and control under NDIS guidelines.
- Well-developed communication and interpersonal skills including the ability to work and communicate effectively with team, participants, their families, allied health professionals, community agencies and other professionals.
- Ability to work in a very busy environment, work independently and as a part of a team.
- Advanced skills in Microsoft Office applications.
Tasks & responsibilities
- Support participants and carers to understand their NDIS Plan, goals and objectives
- Ensure all organisational and administrative reporting and accountabilities are completed accurately and in a timely manner
- Deliver coordination of supports to individuals with an NDIS plan:
- Research, coordinate and manage complex and intersecting supports to suit individuals needs across multiple providers, ensuring client choice & control
- Arrange any assessments needed to determine the nature and type of funding required
- Assist participants to prepare for plan reviews by helping them assess whether they achieved their goals and got value for money for their plan, identify solutions to problems experienced in implementing the plan, and to consider new goals
- Provide responsive, personal centred support coordination service, ensure people with disability exercising choice and control when receiving supports and services
