Support Coordinator – NDIS
Part time – 22.8 Hours per week (Negotiable)
This is an Aboriginal and/or Torres Strait Islander designated position, classified under Section 12 Special Measures of the Equal Opportunity Act (2010). This employment opportunity is only available to Aboriginal and Torres Strait Islander peoples.
About the Role
As a Support Coordinator you will assist participants of the local community to navigate and implement their NDIS plan's by working alongside service providers and allied health professionals.
You will manage your own caseload of NDIS participants, providing them with guidance to develop goals and demonstrate a person-centred approach to all aspects of the role by supporting clients to achieve their goals in line with their NDIS Plans. This includes assessments, educating and empowering participants to exercise choice and control while learning and building skills.
About You
- Tertiary qualifications in Aged Care, Community Services, Case Management or equivalent.
- Experience working in an Aboriginal organisation and/or an understanding of Aboriginal culture and history
- Sound knowledge of and skills and experience in working with people with a disability and frail older people and their carers in the community.
- Knowledge of the NDIS, and the reasonable and necessary legislation, support coordination and brokerage models of service delivery.
- Understanding of the issues faced by Indigenous people.
- Experience working with electronic client data management systems, particularly Community Connect.
- Well-developed interpersonal, communication and computer skills.
- Demonstrated initiative and ability to work without supervision and as part of multi-disciplinary team.

- Valid drivers license, without restriction
