Job description

Part-time, Fixed-term contract (12 months)
Based in Oaklands Park
Support NDIS participants, their families and carers to navigate the system in order to meet their individual goals
Who are we looking for?

We have an exciting opportunity for a passionate and enthusiastic individual who wants to make a difference in the lives of people living with a disability or mental health illness. With excellent communication and interpersonal skills you will have the ability to engage with our customers, building relationships with them, their families or carers. With a sound understanding of the NDIS, disability and community services, you will provide a tailored service pertaining to the needs and goals of your customer.

What can you expect to be doing?
Providing short term intensive support, developing the capacity of individuals, their families or carers to then manage their own supports
Linking your customers to services that build on their strengths.
Establishing partnerships with community services and agencies in order to provide a well-rounded service
Maintaining regular and accurate customer information, documentation and reporting that meet legislative requirements

What do you need to bring?

Tertiary qualification in Disability and Developmental Education or Social Work
Registration with the relevant professional association
Comprehensive knowledge of the NDIS, disability and applicable community services
Outstanding customer engagement and assessment skills, with the capacity to develop a flexible and innovative range of interventions
A willingness to work both autonomously and within a team to provide a well-rounded service
An ability to work toward required productivity rates
