About the Role

The role is located in Canberra.

We expect the successful candidate to operate within the team honestly, authentically, and professionally with a primary goal of providing a holistic service to participants. Our support coordination team do more than just notifying participants of service providers and when their funds are out. We take on complex participants that require ongoing communication and advocacy on the participants behalf to truly support the participant in achieving their identified goals. 

The Support Coordinator position ensures that all participants are receiving the quality service they have agreed with NCC and meets NDIS standards, that the participant is provided with informed choice to receiving quality access to appropriate supports reflective of their identified needs and free from judgement and/or bias. 

Role requirements include: 

  • Act honestly, faithfully and diligently at all times in the best interests of the participant above all else. 
  • Keep up to date with the operational requirements of conducting Support Coordination under the National Disability Insurance Scheme (NDIS) including ongoing professional development. 
  • Assist NCC in maintain the Support Coordination service and system consultancy advice around Support Coordination billing and documentation requirements within a system as required. 
  • Invoices are raised properly, and that Support Coordination is being done within agreed budgets and/or implementing a review the decision as required to ensure best outcome for the participant. 
  • Assist in creating a sphere within NCC for non-bias, participant centred support coordination inclusive of participant advocacy to be available to Canberra and surrounding regions NDIS participants. 
  • Assist NCC and participants in reviewing plans to best maximise budgets and services including liaising with participant supports to gain a full picture to best support participant in this process. 
  • Undertake ‘change of circumstances’, ‘review of decision’ processes under NDIS. 
  • Provide Team Leader information about NDIS Authorised Notifiable Incidents in relation to scope of practice and undertake all reportable incidents on behalf of S/C participants. 
  • Assist the Team Leader in creating, maintaining, and reviewing of appropriate policy and procedural/operational framework
  • Assisting the team leader through Service Coordination NDIS registration/ auditing processes as they present
  • Assist with participant feedback, complaints and/or incidents relating to support coordination and notify NCC management of action and outcome take. 
  • If critical services cannot be assigned to a NCC carer, these services may require relevant Support Coordinator to assist where appropriate. 
  • Support Team Leader in updating NCC on best practice in Support Coordination
  • Provide ongoing advice and assistance to assigned Advocates/Support Coordination and be available to debrief when required
  • Updating Registers as required: Risk Register, Incident/Accident Register, Worker Screening Register, Conflict of Interest Register. 
  • Demonstrate a high level of judgment when faced with complex situations
  • Attend Support Coordination Intake meetings as they arise and advise on capacity and eligibility against policy and procedure and best practice. In the event, policy and procedure need to be adjusted within reason, proposing such changes to NCC management. 
  • Assist Support Coordination team leader to cover clients in the event of other staff leave of absence.
  • Maintain inbox and appropriate storage of participant records 
  • Shared oversight and maintenance of shared inbox 
  • Maintain professional development and ongoing training requirements.

Suitable candidate MUST possess the following skill sets and qualifications:

  • Paid experience working as a Support Coordinator within the ACT
  • Extensive knowledge of the NDIS and pathways to supporting a participant via support coordination. 
  • Drivers License (Mandatory) this role requires travel across ACT
  • Own vehicle & willingness to work throughout ACT
  • Certificate III or higher in Disability/Support Coordination
  • Completed the NDIS Worker Orientation Module (available online from NDIS) 
  • Completed the COVID-19 infection control training Module (available online from Federal Government ) 
  • ACT Working with Vulnerable People Card - ensure you have ticked NDIS & Children activities 
  • Annual Influenza vaccination​​​​​​​
  • COVID 19 vaccination 1 + 2 + 3booster 
  • Valid First Aid Certificate & CPR
  • 100 Points of Identification 
  • 2 Professional referees
  • Ability to start ASAP
