Purpose of Role
Join our dedicated team at My Coast Support and play a pivotal role in empowering NDIS participants. As a Support Coordinator, you will assist participants in understanding, implementing, and utilizing their NDIS plans effectively. Your efforts will link them to essential community, mainstream, and government services, enhancing their ability to maintain relationships, manage service delivery tasks, live more independently, and integrate into the community.
Key Responsibilities
- Plan Implementation and Optimization: Guide participants in maximizing the benefits of their NDIS plans. Negotiate with providers regarding services and costs, ensuring participants can exercise choice and control.
- Stakeholder Engagement: Work closely with participants, their families, and care providers to foster a collaborative environment that respects all parties' contributions.
- Service Coordination: Efficiently action referrals, arrange necessary assessments, and prepare participants for plan reviews. Evaluate the effectiveness of services in achieving participant goals.
- Reporting and Compliance: Develop detailed reports and maintain accurate case notes. Provide regular updates to the NDIA and ensure compliance with all regulatory requirements.
- Community Integration: Explore and connect participants with community and mainstream services to support long-term goals.
Required Skills and Qualifications
- Resilience and excellent communication skills, with the ability to operate independently.
- Professional demeanour, ensuring all interactions are conducted with respect and integrity.
- Strong organizational skills, with a keen eye for detail and the ability to manage multiple tasks efficiently.
- Proven ability to network effectively and build relationships with a variety of stakeholders.
- Experience in the disability sector, particularly with NDIS planning, is highly desirable.
