The Role:

As aSupport Coordinator, you'll play a vital role in helping NDIS participants navigate their plans and connect with the right supports. Your duties will include:

• Maximising NDIS Plans and assisting with budgeting and forecasting
• Facilitating connections to relevant supports and services tailored to individual needs
• Collaborating closely with participants to achieve their NDIS goals
• Ensuring compliance with legislation and codes of conduct
• Monitoring plan outcomes and meeting reporting requirements
• Building and maintaining effective relationships with participants and their families

What We're Looking For:

• Minimum one year of previous experience as a Support Coordinator or, maybe you are a Support Worker wanting to take the next step in your career
• Full working rights in Australia
• Flexibility and willingness to travel around Sydney as needed
• Current National Police Check
• Passion for making a difference in the Disability Sector and helping individuals live their best lives
