The Role
Well Life Services is seeking an experienced Support Coordinator who is flexible, innovative, motivated, and passionate about delivering exceptional services and supports to individuals living with disability. Support Coordinators provide reliable, flexible service linking participants with both mainstream and specialist services. You will ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community. We require someone with energy, enthusiasm and initiative who will thrive in an agile and changing environment. This role offers a flexible hybrid work model, allowing for work from home and the office, and is open to both part-time and full-time candidates.
Position responsibilities.
The Support Coordinator is responsible for the following:
- Working with participants to achieve their goals in line with their NDIS plan.
- Providing ongoing support coordination services to participants in line with their NDIS plan
- Using a coaching approach to assist participants in capacity building
- Maintaining accurate and up to date case notes on participant's progress
- Ensuring Work Health and Safety guidelines are adhered to.
- Engaging with participants, their supports and third-party organizations in a professional manner.
- Undertaking work in line with the organizations policies, procedures, and values.
- Complying with all relevant legislation and codes of conduct.
- Maintaining an up-to-date knowledge of the NDIS systems and practices.
- Maintaining clear professional boundaries with participants; and
- Maintaining an up-to-date knowledge of local services and organizations participants can utilize.
- Providing crisis support to participants; and
- Assessing and managing risk in line with organizational policies and procedures
- Ensure mainstream services meet their obligations (i.e. housing, education, justice, health)
About You
- Demonstrated experience as a Support Coordinator with the NDIS
- Qualification in Disability or Community Services.
- Demonstrated high level interpersonal, verbal and written communication skills.
- Demonstrated ability to work collaboratively and supportively within a team and the ability to work within set timelines.
- Excellent time management skills and ability to prioritise.
- Experience in service development and evaluation including initiating and coordinating quality improvement.
- Excellent written and verbal communication skills; and
- Proficiency in MS Office, Office365 and Excel.
- Proven desire to actively participate in professional development.

- Current Working with Children’s Card
- NDIS Worker Screening Check
- QLD Australian C Class Driver’s License with car insurance and registration.
- Current First Aid and CPR Certificates.
