Client Background:
-Age & Situation: 23-year-old male , transitioning into the community.
History: Institutional care since the age of 15.
Disabilities: Primary disability is schizophrenia
-Independence:Fully independent in daily living activities.
-Behavior Support: A Behavior Support Plan (BSP) is in place, with no restrictive practices.
Interests: Enjoys fishing and is looking forward to engaging in community activities.
Key Responsibilities:
- Deliver tailored support services according to the client's individual plan and preferences.
- Assist with daily activities while promoting independence and respect.
- Facilitate social engagement and activities, particularly those of interest like fishing.
- Ensure the client's well-being, with prompt reporting of any concerns.
- Work independently to meet the client's support needs effectively.
Requirements:
- Experience in support work, preferably with similar client backgrounds.
- Exceptional interpersonal and communication skills for building rapport.
- Dedication to compassionate, respectful, and client-focused care.
- Ability to work autonomously with sound judgment.
- Relevant background checks and certifications in the health or social care sector.
- Valid driver’s license and reliable transportation (if necessary for the role).
