Continuity Care have employment opportunities in Moreton Bay, North Brisbane and South Brisbane regions for experienced Support Workers across:
- Supported Independent Living
- Community Access Support
- In-home Care
The Role:
- Understand and provide appropriate support to individuals living with disabilities and/or mental health conditions.
- Assisting with activities relating to daily living, community support, and improving quality of life.
- Work with the person seeking support, their families, and stakeholders.
- Adhere to organisational policies and procedures.
- Complete all reporting requirements including incident reporting and behaviour support.
- Be empathetic, passionate and committed to provide the best care to each individual.
- As a Support Worker you will be willing to commit to regular and consistent shift schedules as well as irregular hours, including early mornings/evenings, sleepover and weekends.
Skills and Experience:
- Previous experience as a Support Worker, aged care worker or nursing.
- You will possess outstanding skills in interpersonal relationships, communication and resiliency.
- You will be adaptable to working with different individuals including complex needs and challenging behaviours.
- Experience working with and an understanding of positive behaviour support plans is highly desired.
- You will have a strong sense of professional boundaries.
- Be able to complete documentation and reporting as required.
Mandatory Requirements:
- Cert III in Disability or Individual Support is highly regarded
- NDIS Workers Screening
- Blue Card
- Current First Aid and CPR
- Driver's Licence
- Own reliable transport
