About the role

Catholic Homes is seeking an experienced Support Worker Team Leader to cover a 12 month Maternity Leave. This is a full-time position based at our Head Office in Burswood with some metropolitan based travel. Position is to commence ASAP to ensure a full handover for the successful encumbent.

The purpose of this position is to assist in leading the delivery of safe and high-quality care and services to Catholic Homes’ Home Care clients via our Support Worker Team. This position will also provide guidance, mentoring and assistance to Support Workers in their role, assist clients indirectly with their needs including maintaining independence in the delivery of personal care, transport, medication administration, domestic assistance etc.

It is a busy role with a high level of interaction with not only Support Workers, but with the Scheduling Team as you work to together to provide the best fit through high level interactions and training.

The Support Worker Team Leader reports directly to the Home Care Operations Manager.

Key responsibilities of the role include: 

  • Be the go-to for a large Support Worker Team and help manage enquiries around scheduling, policy and procedure.
  • A great memory of Support Workers and clients whilst endeavouring to build strong relations between the two hence developing strong rapport and respect for each other.
  • Supervise, support and assist Support Workers when required, including arranging ‘buddy shifts’ for new staff members and on-the-job assessments/training.
  • Schedule, prepare and facilitate regular staff meetings for metro and regional based staff (Teams meeting)
  • Ensure staff documents are current, up to date and maintain database for compliancy.
  • Monitor and maintain quality documentation and reporting processes submitted by support staff including Incident Reporting and investigation.
  • Contribute information to team around clients' assessment and care plans ensuring information is accurate and current.
  • Recruitment of support staff, induction and orientation.
  • Monitoring of safety and well-being of the support staff.
  • Attend to Support staff in a responsive and caring manner ensuring changing support needs, goals and preferences of Clients are met.
  • Be the conduit between Case Managers, Scheduling and Support Workers ensuring appropriate allocation and utilisation of team.

To be successful in this role you will have/be:

  • Excellent communication skills both verbal and written.
  • Experience supervising and leading a team and can work well within a team environment. 
  • Excellent problem solving, organisational and time management skills with the ability to multi-task.
  • Outstanding level of confidentiality.
  • Ability to maintain a professional and positive outlook whilst at work.
  • Experience working in Community Aged Care and familiar with the Aged Care Standards in a senior Support Worker role with management experience.
  • Holder of a Certificate III in Aged Care or Certificate IV in Training and Assessment (desired but not essential).
  • Current drivers’ licence.
  • Experience in quality assurance requirements for Aged Care.
