Rise is hiring Support Workers to join their Community Aged Care Team. In this role you will support our clients in their homes & assist them to remain independent & in their own homes for as long as possible.
A typical day might look like…
- Follow client support plans.
- Provide personal care to clients.
- Record & report client changes to supervisors.
- Assist clients towards meeting individual goals.
- Follow medication assistance procedure.
- Build supportive & engaging relationships with clients.
- Enable clients to access social opportunities.
- Participate & contribute to meetings.
This role is permanent part-time, working Monday to Friday with guaranteed hours between 8.00am – 1.00pm, however, it will be highly regarded if you are available between 7.00am – 2.00pm. You will be required to drive to client’s homes in either Perth’s upper OR lower North Metro Areas, depending on which area suits you best!
Upper North- Hillarys, Joondalup, Wanneroo, Banksia Grove, Koondoola, Clarkson, Butler & surrounding areas.
Lower North - Karrinyup, Balcatta, Yokine, Morley, Bassendean, Bayswater, Beechboro, Mount Hawthorn, Wembley & surrounding areas.

- Cert 3 in individual support with demonstrated experience working with high care and complex clients.
- Comfortability with providing personal care and essential services.
- Medication trained or willingness to be trained.
- Availability on weekdays between Monday to Friday, 8.00am to 1.00pm (essential).
- Availability on weekdays between Monday to Friday, 7.00am to 2.00pm (desirable).
- Driving licence and own vehicle to use for work purposes.
- Happy to drive around the North Metro area to client’s homes.

Appointments to this position will be subject to a National Police Clearance / requires a driving licence and reliable vehicle to use for work purposes / requires a pre-employment medical assessment.
Costs for new checks will be covered and/or reimbursed by Rise.
