About the Role
We are seeking an experienced Care Services Coordinator or Support Worker in Aged and Disabled Care. In the role you will lead a multidisciplinary team to deliver person-centered home-based services to older people, people with disabilities and their carers to meet the requirements of relevant policies, procedures, service standards, budgets and targets.
This will involve:
- Oversee the day-to-day operations of Community Support Services.
- Lead, support and manage the Community Support Services team.
- Participate in strategic planning for services.
- Management of appointed contractors.
- Establishing networks with both internal and external stakeholders to enhance and promote quality service delivery.
- Ensuring monthly and annual targets are met.
We are looking for compassionate individuals with a desire to help the community in a meaningful way.
The successful candidate will have:
- Tertiary qualification in Community Services, Allied Health, Human Services or a related field.
- Lesser formal qualifications with substantial experience also applicable.
- Previous experience in working with older people and people with disabilities within a home or community setting.
- Demonstrated ability to lead, supervise, train and support staff.
- Previous experience within assessment, planning and care coordination for clients and demonstrated commitment to person-centred care.
- Excellent verbal and written commnunication skills.
- Current Police Check and Working with Children Check.
- Current Victorian Driver's Licence.
- Ability to speak a second language is desirable.
