We have an exciting opportunity for a part time (65.2 hours per fortnight) Out of Home Care (OOHC) Team Leader to lead our team who provide support within a managed case-plan to families and carers.
This position is based in our Launceston office and will report to and be supported by the Out of Home Care Program Manager currently based in the NW.
The OOHC Team Leader will provide high-level quality care and support to the OOHC team and will help to promote the safety, stability and wellbeing of vulnerable children, young people and their families, and to build child, family and community capacity and resilience.
We are looking for a person with skills, experience and talent in therapeutic / trauma informed care who hold a Certificate III in Community Services or above and who align with our values of Compassion, Hope, Respect, Integrity and Support.
Key Responsibilities:
- To lead and supervise a team of OOHC support workers who provide 24/7 carer support.
- Maintain a high standard of in-house case management for the people in the house. This includes participant files, case notes and monthly reports.
- Liaise with Child Safety Services, program deliverers and service providers as required.
- Assist with ensuring children in care are given appropriate placements that will support their development and meet their needs in a safe and secure environment.
- Assist with ensuring that the placement goals established with the child (where appropriate), the foster carer and the case manager are achieved.
- Ensure an active and collaborative care team approach which bring together cross-sector organisations and programs with a focus on a common agenda that results in long lasting change for children in care.
- Ensure a high level of quality service and support is offered to foster carers.
- Lead recruitment activities for Out of Home Care Carers, including facilitating information sessions and external promotions.
As the OOHC Team Leader, you will have:
- Previous experience in a relevant industry, service or an equivalent level of expertise and experience to undertake the range of activities required.
- A Certificate III in Community Services or are working towards acquiring one.
- Knowledge of the Social, Community, Home Care & Disability Services Industry (SCHADS) Award & NDIS.
- Familiarity with Child Safety Services and legislative requirements.
- Strong communication, organisational and administration skills.
- An ability to align with our organisational Christian heritage and practice Glenhaven's values.
- A caring nature, be patient, supportive, resilient, and passionate about people and your role.
- A willingness to be available to take a call from Glenhaven's managers or on-call staff after hours if needed.
- A current First Aid certificate, Working with Vulnerable People (NDIS-endorsed) card and current Australian Driver’s licence.
- A current Nationally Coordinated Criminal History police check or happy to submit to one.
- Strong computer-based skills including use of Microsoft suite of products.
