As a Welfare Officer, you will work with the Kids Rehab WA team and deliver comprehensive social welfare services to patients and families as part of our overall mission of providing the highest standard of healthcare and disability support to families in need of paediatric rehabilitation services, while striving to enhance participation and quality of life for our young people.You will assess and prioritise the welfare needs of families of children and adolescent, including living situations, financial resources, and support networks, and provide direct assistance through facilitating access to benefits and other practical needs.Your key responsibilities will include:
- Conducting assessments to identify the practical needs of patients and their families and undertake appropriate interventions.
- Acting as an advocate for patients and their families, referring to other disciplines and/or agencies in conjunction with Social Work staff as required to ensure coordinated care and support for patients and families.
- Educating patients' families about existing community supports, entitlements (such as Centrelink), and how to access relevant services including crisis assistance
- Maintaining confidential medical records, developing and maintaining the department’s community resource and welfare database and maintain relevant statistics as required.
- Participating in approved quality and research projects and education programmes.
