About the role:

· Based at Kawana Waters Care Community, Birtinya

· Monday - Friday, 9am - 5pm

· Lead the Meaningful Life program in the Care Community

· Develop opportunities for meaningful engagement, purposeful participation, social events and activities for all residents

· Drive Opal HealthCare initiatives including Meaningful Mates and Helping Hands

· Curate programs to support residents’ physical, emotional and spiritual wellbeing and overall quality of life

· Develop resident care plans and complete key lifestyle assessments

· Work together with clinical teams and allied health professions provide a tailored approach for each resident to aid their mobility and independence

· Coach care team members to deliver meaningful engagements between like-minded residents

· Liaise with residents to develop activities and engagements that meet their cultural, spiritual, social and physical needs and preferences

· Engage with residents’ families to encourage visitation and socialisation

· Demonstrate Person Centred Care principles approach

· Tertiary qualifications desirable
