Position Summary:
The Case Manager is responsible for working with at-risk youth to engage them in pro-social activities, outreach programs and other early intervention and/or diversionary measures to achieve positive behaviour change. Provide case management services to youth at-risk of entering, or having entered, the justice system and support the progress and development of other team members.
To be responsible for the provision of a culturally appropriate service to all individual clients.
Key Responsibilities:
• Establish early intervention, diversionary case management plans that will link young people to a range of services and activities and divert youth from the Justice System.
• Be available for irregular hours if required.
• Travel to remote Indigenous communities on day trips when required.
• Ensure relevant Youth Diversion forms, templates and Kalano documentation completed efficiently in accordance with regulations, including assessment and signed informed consent paperwork to access other relevant information.
• Monitor and record all client needs and progress, including daily appointments.
• Coordinate client appointments, business and recreational activities / outings as required, including cultural activities.
• Liaise with other stakeholders, agencies and service providers in a prompt and professional manner.
• To develop an ethical and confidential working relationship with individual clients to provide the appropriate support.
• Participate in supervision and case reviews.
• Participate in planning and program reviews as required.
• Participate in quality assurance activities and statistical duties as required.
• Undertake other duties as required by management.
Qualifications & Experience:
• Certificate III in Community Services (or equivalent) is desirable.
• Demonstrated awareness of and sensitivity to Aboriginal culture and history and knowledge of issues impacting on Aboriginal children and families, youth and adults.
• Experience with case management, youth work or community services is desirable
The successful applicant should posses the following;
• Proficiency in the use of electronic information systems for the maintenance of progress notes and reports.
• Ability to work in a team as well as unsupervised.
• Demonstrated ability to prepare written reports/notes of a sound standard.
• Cross cultural awareness and effective communication skills with both participants and stakeholders from diverse cultures.
• A positive attitude with a high standard of work ethic.
• Willingness to undertake further training as required.
• Valid NT Drivers Licence, Valid Ochre Card and willingness to undergo a National Police Check
Renumeration package:
Employment Type: Full Time
Salary Range: $70,000 - $75,000 p.a (pro-rata)
Entitlements/Benefits: 6 Weeks Annual Leave + 2 Weeks Sick Leave + 2 Weeks Ancillary Leave + 1 RDO per Month
Location: Kalano’s Big River's Child & Family Centre – Katherine, NT.
Hours of work: 37.5 hours per week – 7.5 hours per day [Monday to Friday | 08:00am – 4:30pm]
