What you'll be doing
The Senior Chronic Care Coordinator - Shoalhaven will support the Manager of Aboriginal Chronic Care (ACCU) to implement the Models of Care for Aboriginal people in Shoalhaven and Milton Ulladulla hospital and Aboriginal people at risk of developing a chronic disease living within the Shoalhaven Region (including Jervis Bay Territory). The position will support the implementation, monitoring, and evaluation of Aboriginal Chronic Care initiatives involving health promotion and education to meet the needs of the Local Aboriginal community. While fostering a multi-disciplinary community health team approach. The role will be required to work in a collaborative and integrated approach with hospitals and other chronic disease services to ensure ACCU programs meet clinical and national accreditation standards.
This position will support Aboriginal people who access programs and services within the Shoalhaven hub including Shoalhaven District Memorial Hospital, Milton Ulladulla Hospital and Jervis Bay Territory outreach locations.
This position is open to Aboriginal and Torres Strait Islander applicants only in accordance with Clause 26 of the Government Sector Employment Rules 2014, applicants must demonstrate Aboriginality in addition to addressing the selection criterion.
All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (2 doses) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available.

Selection Criteria * Must be of Aboriginal and/or Torres Strait Islander descent (NB applicant's race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW) and have demonstrated knowledge of current Commonwealth, State and Local Aboriginal health priorities and ability to effectively and sensitively liaise and effectively communicate with local Aboriginal and Torres Strait Islander people and communities and agencies

  • A tertiary health qualification in a health promotion, public health, or clinical discipline, and/or highly relevant work experience, and willingness to undertake further education, as required.
  • Demonstrated ability to engage and work autonomously and effectively under limited supervision with a range of agencies, including working within a multidisciplinary team.
  • Demonstrated high-level verbal and written communication skills with an ability to negotiate and work effectively with a range of health professionals/service providers to ensure coordination of care and appropriate allied health and social service referrals.
  • Demonstrated competence in; maintaining accurate and appropriate records; produce routine written correspondence; prepare reports and present information relevant to preparation of more complex documents, such as ability to research, analyse and collate data.
  • Demonstrated ability to evaluate health programs with a commitment to quality improvement initiatives and ethical practice.
  • Demonstrated high level verbal and written skills to effectively ensure Management of programs and organisational skills within health including; an understanding of accountability mechanism and communication skills required for program or service success.
  • Current driver's license Class C and ability and willingness to travel within the LHD for work purposes.
