Position Title: Aboriginal Hospital Liaison Officer
(1x Permanent Full Time, 1x Temporary Full Time – 12 months contract)

About The Role
Provide flexible, holistic, and culturally sensitive health care to Aboriginal patients in the hospital setting. Provide support and advice to community to achieve better health outcomes and better access to health services for Aboriginal people. The role performs a range of primary health care duties for the community in which they work under direct or indirect supervision at more experienced years.
The Aboriginal Hospital Liaison Officer is a vital conduit and advocate for all Aboriginal people presenting to the hospital and provides support as required to patients and their families. Support will be provided in all health facilities across the Central Coast.
Key Accountabilities
· Demonstrated commitment to Caring for the Coast vision, goals, and strategies, with demonstrated behaviours which align with the NSW Health CORE values and CCLHD Values and behaviours charter.
· Provide culturally appropriate health care for Aboriginal people in line with the NSW Ministry of Health - Aboriginal Health Worker Scope of Practice across all sites of the CCLHD
· Support and inform Aboriginal people and their families to ensure an understanding of clinical interventions and service delivery processes than enable informed decisions in relation to their health needs.
· Work co-operatively within a multidisciplinary team to ensure an integrated and comprehensive approach to the provision of care to the Aboriginal Community. Participate in the provision of health promotion programs, appropriate referral, advice, and information including basic health screening and monitoring.
About You
· Aboriginality: Applicants for this position must be of Aboriginal descent through parentage, identify as being Aboriginal and be accepted in the community as such. Exemption is claimed under Section 14 of the Anti-Discrimination Act.
· Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification in the area of care in which the Aboriginal Health Worker works.
· Demonstrated computer skills utilising the CCLHD platforms and ability to maintain patient/client records, provide statistical data and provide reports to management as required. Demonstrated ability to work unsupervised, use own initiative and achieve deadlines with minimum supervision.
· Demonstrated knowledge and understanding of the key policies and issues that impact on Aboriginal Health within the Central Coast LHD.
