80 hours per fortnight with monthly ADO

About the Role

The Aboriginal Hospital Liaison Officer (AHLO) is responsible for supporting Aboriginal and Torres Strait Islander (ATSI) patients who are dealing with health related issues across the acute/sub-acute/community and Mental Health settings at Grampians Health.

This position may be required to work across various health settings in the Grampians Region. The AHLO assists staff to provide culturally safe care to ATSI patients and families, and works in close collaboration with Social Workers, other health professionals and Ballarat and District Aboriginal Co-op (BADAC), to provide best outcomes(in line with ICAP strategy).

The role aims to strengthen the relationship between Grampians region ATSI communities and Grampians Health mainstream and Mental Health Services through increased awareness of cultural, emotional and social issues affecting the ATSI communities in the Grampians region. The role may be required to visit Ballarat, Stawell, Edenhope and various locations within the Grampians Region.

Key Responsibilities

• Provide specialist cultural assessment, intervention and advocacy for GH clients who identify as Aboriginal and/or Torres Strait Islander 
• Facilitate the smooth transition of patients through the health service system.
• Effective discharge planning
• Liaise with Ballarat and District Aboriginal Co-op (BADAC) and other community controlled and Government departments to facilitate care planning
