Meracare Aged Care is currently looking for a experienced Part-Time AN-ACC Assessor/Coordinator to work at our family owned homes in Frankston, Carrum Downs and Wangaratta. Meracare comprises of three 60 bed facilities where we work with our residents to provide a person-centered care approach and current best practice strategies to enhance the lives of all the residents.
The successful candidates are required to:
- Use your great clinical skills to provide comprehensive clinical assessments that will drive care
- Work as part of a multidisciplinary team
- Provide assessment, planning, implementation and evaluation for resident care
- Prioritise tasks effectively
You must have:
- Experience as an AN-ACC Coordinator
- Demonstrated experience in aged care or a related area of health care, including working with people with dementia
- Relevant tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy with unrestricted AHPRA registration.
- Ability to conduct clinical assessments using a range of assessment tools.
- Strong organizational skills and the ability to work independently and as part of a team.
- A genuine desire to improve the lives of older Australians.
- Your own vehicle as travel will be required between our homes.
