This is a rare opportunity for a dedicated Behaviour Support Practitioner to join our ACT team. You'll be working in a multidisciplinary model with a wide range for allied health professionals to understand our customers' needs and goals to develop and implement a tailored support plan. You'll develop positive and collaborative working relationships with stakeholders and possess an in-depth knowledge of Positive Behaviour Support principals. This role will see you apply your excellent communication and time management skills to effectively manage your case load.

You'll have access to a diverse range of professionals within the Behaviour Support team (access to advanced and specialist practitioner supervision). Key Responsibilities:- Assist customers, their families and support teams to identify their needs through assessment and individual planning, to develop meaningful goals and practical strategies to support customers to meet their goals in their home and community.
- Work in a multi/trans-disciplinary model with a range of allied health professionals to provide a holistic and integrated approach to supports.
- Provide clinical knowledge, expertise and support to individuals, families and support staff.
- To develop and implement individually designed customer related support plans within specified timeframes.
- To handle complex case management.
- To plan, deliver and evaluate Behaviour Supports relating to the individual's needs in line with relevant legislative and ethical requirements.
- To plan, implement and review training and support plans for customers, support staff, families and carers in positive Behaviour Support and specialist topics as required.
