We are a small, community-based organisation and have been providing support for people in our community for over 40 years. Charlestown Caring Group is a registered NDIS provider and a Commonwealth Home Support Package provider under My Aged Care.
Our team of dedicated Support Workers provide support to our clients and participants in the Lake Macquarie, Newcastle and Lower Hunter, to live safely and independently in their own homes and achieve positive outcomes.
We are currently searching for a problem solving, outside the box thinker with excellent time management skills, to join our team on a 3 month temporary relief contract with the potential for extension. The contract will be for 24 hours per week, with the days and times to be negotiated with the successful candidate.
About the role:
As a member of Charlestown Caring Group’s valued Care Coordination Team, you are an experienced, dedicated, and passionate coordination superstar.
As a Care Coordinator you will be responsible for delivering care and services which are person centered and enablement focused in accordance with Charlestown Caring Group policies and procedures, the NDIS Practice Standards and the Aged Care Quality Standards. You will also be responsible for developing and maintaining effective partnerships with colleagues and external stakeholders to facilitate outcomes for our clients / participants.

About You
- A strong empathy to working with older people, people with disability and their carers.
- Kind, friendly and respectful toward the elderly, people with a disability, their families and carers.
- Experience in the intake, assessment, and review processes.
- Experience in support planning with person centered goal setting and assisting people to meet their goals.
- Excellent customer service and communication skills
- In depth knowledge of community Aged Care and Disability services and experience navigating the Aged Care and Disability systems.
- Have a good working knowledge of MS Teams, MS Office and the ability to learn new systems with ease
Essential criteria:
- Diploma or Certificate IV in Community Services Coordination.
- NDIS Worker Check (or willing to obtain)
- Working with Children Check (or willing to obtain)
- NSW Driver’s License with your own comprehensively insured vehicle
- Current First Aid Certificate
- COVID 19 Vaccination including Booster and proof of vaccination status
- Influenza Vaccination and proof of vaccination status
- Willing to work a rotating on call roster
