Care Coordinator/Counsellor

Yulu-Burri-Ba is currently seeking a highly motivated Care Coordinator/Counsellor to join our Family Wellbeing Service!

The Yulu-Burri-Ba Aboriginal Corporation for Community Health (Yulu-Burri-Ba) provides a safe, friendly, confidential, and culturally appropriate Aboriginal and Torres Strait Islander Primary and Social Wellbeing Health programs in line with community needs. It is our vision to empower our Aboriginal and Torres Strait Islander families and community to achieve excellence in their healthcare outcomes.  Yulu-Burri-Ba has clinics at Capalaba, Dunwich on North Stradbroke Island and Wynnum.

• Have a Queensland Working with Children Blue Card (Paid Blue Card)
• Current registration with recognised professions

As a key member of the Yulu-Burri-Ba Social Health team, the Family Wellbeing Care Coordinator will:

Provide a Family Wellbeing Care Coordinator Role by Developing Trusting Relationships with Families

• Effectively engage with Aboriginal and Torres Strait Islander families in support services and conduct initial comprehensive assessments, utilising narrative therapy and family-led approaches to identifying the real and genuine needs of a family. The family-led assessment approach will spearhead into the development of a ‘Single Family Care Plan’, with corresponding goals. The Family Wellbeing Care Coordinator will act as the main ‘point-of-contact’ for the family and work in partnership with the family to help facilitate supports in achieving their goals

Deliver Coordinated Care for the Management of Holistic Health and Social Needs:

·         Effectively manage and coordinate health and social support services to families, based on the Family Care Plan. The Family Wellbeing Care Coordinator will act as a navigator of the health and social services system to improve access and quality of service responses to the family. The role will lead internal and external case planning activities with other agencies

Provide Social and Emotional Wellbeing Interventions and Assessments

• Provide wellbeing interventions using established techniques and methodologies of proven effectiveness from a sound evidence base. 
• Appropriately respond to critical incidents, providing wellbeing first aid to patients in distress.
• Conduct comprehensive wellbeing holistic assessments for the purpose of evaluating risks, identifying client needs and guiding treatment delivery plan.  
• Ensure clinical and administrative standards are maintained, in accordance with case management and record keeping processes. 

Remove Barriers to Family Wellbeing and Provide Family Advocacy:

·         Deliver therapeutic brief interventions to encourage families in achieving their goals and provide crisis response care as required. The role will operate heavily through a community outreach/in-home engagement approach to remove any access barriers to families. The Family Wellbeing Care Coordinator will provide direct support alongside the family in resolving social welfare matters (i.e. Centrelink, housing etc.). The role will also act as a family advocate; ensuring families are receiving best quality services and are actively involved in all decisions that affect their lives and treated with respect, dignity, and empathy.

Essential Requirements:

• Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals.
• Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management.
• Ability to work with diverse people and presentations in providing case managed social work driven supports.
• Demonstrated ability to record accurate and consistent case notes.
• Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.
• Demonstrated experience in overseeing and coordinating case management processes and procedures.
• Demonstrated ability to apply a variety of evidence-based, trauma-informed and strength-based approaches to counselling.
• A willingness to provide social work supports and engagement to patients outside of a clinic environment.

Qualifications/experience

• Bachelor qualification in either Social Work/Human Services/Behavioural Science/Psychology/Criminology/Allied Health/Occupational Therapy/Sociology/Public Health or relevant field.
• Previous experience in a family support and or advocacy role.

PRACTICAL REQUIREMENTS

• Travel across Yulu-Burri-Ba clinics (Capalaba, Wynnum and North Stradbroke Island) may be required
• Current C Class Driver’s License (Qld) – essential
• Current Cardiopulmonary Resuscitation Certificate (CPR)
• Current First Aid Certificate (If applicable for position)
• Satisfactory current Criminal History Check or willingness to obtain
• Proof of Immunisation Status – or willingness to obtain as a matter of priority
• Current Blue Card (working with children and young people)
