What You'll Do:
As their Care Manager, you'll be at the forefront of shaping their care delivery model and inspiring a team dedicated to excellence.
Your responsibilities will include:
- Providing leadership to their care team, fostering a culture of compassion, collaboration, and continuous improvement.
- Overseeing the development and implementation of individualised care plans, ensuring they reflect their commitment to holistic wellness and meet the unique needs of each client.
- Serving as a liaison between clients, families, healthcare professionals, and their care team, facilitating communication and collaboration to optimise outcomes.
- Monitoring the quality and effectiveness of care delivery, implementing best practices, and driving innovation to enhance the client experience.
- Providing mentorship, guidance, and support to their care team, empowering them to deliver exceptional care with empathy and professionalism.
About You:
We're seeking a dynamic and compassionate leader with a proven track record of success in NDIS management, or healthcare leadership roles.
The ideal candidate will possess:
- A background in NDIS, nursing, healthcare, social work, or a related field, with demonstrated leadership experience.
- A strong understanding of NDIS funding models, compliance, and legislative requirements.
- Exceptional communication, interpersonal, and problem-solving skills.
- A passion for holistic wellness and a commitment to person-centred care.
- The ability to inspire and motivate others, fostering a culture of collaboration, innovation, and continuous learning.
- Experience in care coordination, case management, or healthcare administration is preferred.
