The Role
We are looking for an experienced Care Manager who is passionate about a person-centered approach to care, to manage the care of residents in our beautiful 60 bed facility in Maryborough - PresCare Groundwater Lodge. This is only a short 29 minute drive from Harvey Bay.
This role reports directly to the Residential Service Manager and is responsible for ensuring residents are provided with safe, high quality care that aligned to our client’s model and philosophy of care, the Aged Care Quality Standards, regulatory requirements, and professional practice guidelines.
What you will do
The Care Manager is responsible for ensuring:
- Residents are engaged and enabled to live their best life
- Assessment, care planning and care evaluation are managed effectively
- Safe, high quality, personal and clinical care
- Resident classification is managed effectively in accordance with Commonwealth guidelines
- The care team is developed, supervised and supported
- Comply with Aged Care Quality and Safety Standards.
- Clinical indicators are monitored, managed and reported
- At least two 2 years experience in a care management role in residential aged care
- Be passionate about delivering a person-centered approach to care
- Excellent communication skills (written, verbal & interpersonal)
- Demonstrated ability to work autonomously
- Registration with the AHPRA as a registered nurse is essential
- Currently hold and maintain a Police Certificate
- Hold and maintain NDIS Worker Checks
