Position Profile: The Case Management Officer provides assessment, integrated care and Case Management of metropolitan based clients with complex psycho-social needs and challenging behaviours presenting a high-risk of exposing others to the risk of HIV infection. This role provides consultancy advice and professional development on Case Management to country regions; provides education, counselling and psycho-social support on HIV to clients and their significant others; and liaises with a range of service providers and clinicians to plan, implement and monitor care plans for clients. In addition you will investigate allegations of HIV positive persons knowingly infecting others as well as providing State-wide training and development to workers in primary health care and public health services on Case Management plans and case support.
