Empowering Minds and Development is a Specialist Case Management team committed to implementing strength based, trauma informed and culturally sensitive practices. Our services focus on addressing barriers and reducing complexity in the NDIS space while assisting our clients in connecting with supports while building capacity and resilience in their communities.
In the role of a NDIS Case Manager, you will be required to establish strong rapport and working relationships with a range of stakeholders to implement a range of support services in line with NDIS plans.
Key responsibilities are:
- Engage in evidence-based best practice case formulation, assessment, reporting, and interventions for clients identified with complex needs.
- Manage an ongoing and long-term case load of clients, including achievement of KPIs.
- Undertake informed referrals to appropriate allied health professionals and/or service providers.
- Initiate and maintain productive relationships with clients, stakeholders, informal supports, and service providers who may present with differing priorities.
- Manage client's NDIS plan to ensure services are being delivered within their allocated budget.
- Develop and provide reports on outcomes and success indicators, within the required timeframes.
- Develop strategies which will address the identified needs of the client in consultation with their representative/s where appropriate, in accordance with funding guidelines.
- Support clients and their stakeholders, to understand the supports in their plan, navigating NDIS processes, identifying and accessing mainstream community and funded supports.
- Manage accurate record keeping and the security and confidentiality of records and information in accordance with Information Privacy Standards.
Qualifications & Experience:
- Knowledge and/or the ability to rapidly gain an understanding of NDIS, disability and community services sector.
- Exceptional customer service, negotiation and rapport building.
- Strong interpersonal skills and high levels of resilience.
- Excellent writing, comprehension, and verbal skills.
- Ability to interpret information guides, processes, policy documents as well as clinical reports.
- Demonstrated skills and knowledge in the area of planning, implementing, reviewing and evaluating effective service plans.
- Have creativity, a positive attitude, eagerness to learn and willingness to adapt.
Desirable:

- Degree in Social Work, Psychology, Counselling, Criminology or related field.
