About the role
We are seeking a dedicatedCase Manager! You will work within a crisis accommodation residential program to provide intensive case management outreach support (2 sessions per week) to people who are homeless or at risk of homelessness to access affordable accommodation.
What is Gateways?
Gateways is a program that provides short-term crisis accommodation and intensive support to all persons residing within the program. Gateways seek to assist and identify issues surrounding homelessness and make relevant referrals to professional support.
Responsibilities include
- Client referrals are assessed in accordance with program criteria
- Clients receive multiple weekly contacts and have safety / risk plans developed where required
- Individual case, safety and or private rental action plans are developed in collaboration with clients and include a range of interventions
- Victorian Housing Register, community housing and or other housing related applications are completed and submitted in a timely manner
- Clients assisted to access support services and establish community / specialist supports
This is a permanent full-time position, based in Croydon, VIC. Salary and conditions are in accordance with SCHADS Award Level 5. You will be rostered Monday - Thursday 9am - 5:06pm, and on Friday's 11:30am - 7:36pm.
What you will have
- Relevant tertiary qualification in the area of Social Work, Social Welfare or related field
- Demonstrated experience with accompanying highly developed skills in working with persons experiencing homelessness and who display a range of highly challenging behaviours (essential)
- Knowledge of service systems, in particular youth, housing and homelessness, Child Protection, AOD, Family Services and Mental Health
- Experience in applying Case Management principles and practice
- Demonstrated skills engaging homeless persons and willingness to assist in provision of support in areas of domestic violence, drug & alcohol, mental health, living skills and parenting
- A Current Victorian Driver's Licence
