The core mission of Anglicare (NSW South, West & ACT) is to break the cycle of intergenerational poverty for children, young people and families. We particularly focus on serving vulnerable communities in the ACT and regional, rural NSW.
The opportunity
As the Case Manager, you will provide effective client-focused and outcome-based case management while supporting families and individuals. This position is within our Housing, Child, Youth, and Family Services, based in Yass.The position provides service delivery four days a week, with possible rostered on-call duties that include some weekends.
Some key duties include providing case management to clients experiencing complex issues, client interviews and working with those clients to identify and address their needs, home visits, referrals and advocacy, safety planning, support and mentoring, and working with external service systems to promote connection for clients.
What's needed
You'll be drawing upon your experience and knowledge in trauma-informed care, as well as your experience in working alongside Aboriginal communities, so top-level communication and culturally competent skills will be needed. You will also need a minimum diploma qualification in Community Services or a related discipline; degree qualifications are preferred.
