The Tidda program
Tidda supports adult women living with undiagnosed or unsupported disability successfully reintegrate into the community after leaving prison. Tidda works with adult women to navigate the justice system and to ensure participants have access to the skills, knowledge, support and resources they need to live healthy, independent and crime-free lives.
The Tidda team uses a multidisciplinary approach to ensure continuity of support during each participant’s journey towards recovery, reintegration and independence.
The Role:
In the role of Case Manager, you will support individuals with undiagnosed or unsupported disabilities, from low-socio economic and disadvantaged groups, as well as clients with a history of offending and re-offending. Tidda Case Managers work with people who may be living in prison or have offended in the past and are now living in community.
Case Managers maintain relationships with stakeholders, coordinate medical assessments/historic medical evidence, facilitate successful NDIS application submissions and provide ongoing support after funding is attained.
Tidda Case Managers combine person-centred, culturally competent, trauma-informed practice with an understanding of NDIS processes the and justice system to bridge gaps in support and ensure clients can achieve long-term positive outcomes for themselves.
What you’ll need:
Case Management experience (>1 years).
A Diploma in Community Services, or an equivalent or higher qualification in human services.
Exceptional written and verbal communication skills.
Strong ethics, values and knowledge of the rights of people with disabilities; the concepts of social role valorisation, quality of life and self-determination.
Readiness to work in a multidisciplinary team.
Other Employment Requirements:
Must hold Australian Citizenship, or be a permanent resident of Australia.
West Australian Drivers Licence.
National Police Clearance.
NDIS Workers Screening Check.
Working with Children Check.
First Aid Certificate.
