About the Role
Homelessness Services operates within the WA Division of The Salvation Army Southern Territory. The Beacon accommodates individuals and couples who are experiencing homelessness. Support is provided to clients, through a case management framework that incorporates a person centred and strengths-based approach.
This Case Worker position will work with individuals in crisis to develop and implement case plans that promote their best interests and to ensure their safety and appropriate development.
This position provides case management support to clients throughout their stay at The Beacon
We are seeking Full time permanent Case Manager, based at Northbridge, WA.
Salary and conditions are in accordance with the SCHADS Level 5 Award.
Key Responsibilities
- Work with chronically homeless individuals within the region.
- Engage with these individuals to develop self-focussed case plans.
- Work with parents to enhance their parenting skills and address issues contributing to their homelessness.
- Work as part of a team along with case managers and other support workers to achieve the best outcomes for their situations.
- Establish effective relationships with clients that encourage self-determination and participation.
- Provide case management support to clients in accordance with TSA case management models and standards of practice.
- Ensure ongoing assessments of the client's issues, needs and circumstances from initial intake to exit.
- Ensure ongoing client risk assessment and pro-active flagging of concerns to Operations Coordinator.
- Develop individual case plans in collaboration with clients and include a range of interventions that address the needs or goals of clients.
About You
- A minimum Diploma Level in Community Services or Mental Health and or related experience in the industry.
- A national police record check is required.
- An entitlement to work check is required.
- Current WA Drivers licence.
- Current First Aid Certificate with CPR
